This form is for students that have an ERROR on their schedule.
Some examples of errors are:
- You already passed a class and it is on your schedule (again)
- You have 2 of the same classes on your schedule
- You are missing a class such as English
At this time we will not be honoring teacher changes. This is a REQUEST only, no changes are guaranteed.
You will be notified via email if your request is approved or denied.
All requests for Fall semester must be submitted by August 13, 2021.
